They include family history, coronary artery calcification score, high sensitivity C-reactive protein (hs-CRP), ankle brachial index, lipoprotein subclasses and particle concentration, lipoprotein(a), apolipoproteins A-I and B, fibrinogen, white blood cell count, homocysteine, N-terminal pro B-type natriuretic peptide (NT-proBNP), and markers of kidney function. [48] [49] Little is known about the relationship between work and cardiovascular disease, but links have been established between certain toxins, extreme heat and cold, exposure to tobacco smoke, and mental health concerns such as stress and depression. [50] Density-Dependent Colour Scanning Electron Micrograph SEM (DDC-SEM) of cardiovascular calcification, showing in orange calcium phosphate spherical particles (denser material) and, in green, the extracellular matrix (less dense material) [51] Population-based studies show that atherosclerosis, the major precursor of cardiovascular disease, begins in childhood.

The clear link between job stress and cardiovascular disease was scientifically demonstrated 15 years ago by Karasek and Theorell (Karasek and Theorell 1990) and has since been confirmed by numerous other investigators using their demand/control model and it is essential to emphasize the importance of this approach. Anitschkov never concluded from his experiments that cholesterol in the diet caused atherosclerosis in humans, however. It is less useful and not recommended for diagnosing angina pectoris(5).

Adherence to a Mediterranean diet, with a relatively large amount of fruit and vegetables, has been shown to significantly decrease the risk of total mortality and mortality from cardiovascular diseases. 53 In older people from several European countries, lower mortality was related with greater adherence to a Mediterranean diet. 38 Overall, about 10-30% lower risk of all cause mortality was found in most prospective studies comparing highest with lowest fruit and vegetable consumption. 4 7 8 9 11 49 A small study (n=713) that used total serum concentrations of carotenoids as biomarkers of fruit and vegetable consumption further supported a risk reduction in mortality, with a hazard ratio of 0.50 for women in the highest third of serum concentration compared with the lowest third. 5 The findings from our meta-analysis were consistent with a recent study showing that a lower consumption compared with five servings of fruit and vegetables a day was associated with higher mortality in a dose-response manner. 44 For the reduction in total mortality, we found a threshold of around five servings a day of fruit and vegetables, after which the risk of death did not reduce further.
